Step 01

Confirm Your Eligibility

Step 02

Complete a SC Department of Health-approved EMT Course

Once you meet all eligibility requirements, the next step is to enroll in and complete a state-approved EMT training course. All EMT training must be approved by the South Carolina Department of Health and Environmental Control (DHEC), Bureau of EMS.

You can find programs at fire department academies, community colleges, and local EMS training institutes. Pass the NREMT Cognitive Exam

This test evaluates how you react during emergencies, not just your knowledge. The NREMT cognitive exam uses computer-adaptive testing (CAT) format, meaning the difficulty level of each question depends on how well you answered the one before it. As of April 2025, the National Registry has updated the course domains. The new courses represent the current patient care needs.The test may include 70 to 120 questions covering the following domains:

  1. Scene Size-Up and Safety
  2. Primary Assessment
  3. Secondary Assessment
  4. Patient Treatment and Transport
  5. Operations

Note: Items related to pediatric patient care are integrated throughout all domains.

Step 04

Apply for South Carolina EMT Certification

After completing the EMT training and passing the NREMT cognitive exam, apply to the South Carolina EMS Association through their online portal.

  • Current CPR certification
  • NREMT cognitive exam results
  • Background check and fingerprint clearance
  • Application fee

Once your application is reviewed, you will receive your official South Carolina EMT license.

Step 05

Maintain Your EMT Certification

SC EMT certification is valid for two years. To renew, you must:
