Step 1: Confirm Your Eligibility

Before enrolling in training, you must:

  • Be at least 18 years old
  • Have a high school diploma or GED
  • Hold a current CPR certification (Healthcare Provider)
  • Be physically and mentally fit to perform the EMT tasks
  • Pass a criminal background check
  • Meet pre-screening criteria such as math and reading comprehension

Once you meet these requirements, you can start official training.

Step 2: Complete a SC Department of Health-approved EMT Course

Once you meet all eligibility requirements, the next step is to enroll in and complete a state-approved EMT training course. All EMT training must be approved by the South Carolina Department of Health and Environmental Control (DHEC), Bureau of EMS.

You can find programs at fire department academies, community colleges, and local EMS training institutes. You’ll be taught a blend of classroom learning, field/clinical experience, and hands-on skills lab. After completing your coursework, you can move on to the next step: the NREMT cognitive exam.

Step 3: Pass the NREMT Cognitive Exam

This test evaluates how you react during emergencies, not just your knowledge. The NREMT cognitive exam uses computer-adaptive testing (CAT) format, meaning the difficulty level of each question depends on how well you answered the one before it. As of April 2025, the National Registry has updated the course domains. The new courses represent the current patient care needs. The test may include 70 to 120 questions covering the following domains:

  1. Scene Size-Up and Safety
  2. Primary Assessment
  3. Secondary Assessment
  4. Patient Treatment and Transport
  5. Operations

Note: Items related to pediatric patient care are integrated throughout all domains.

Here’s the reality: the best way to study for the NREMT is not through more flashcards. You need:

  • NREMT practice exams that simulate actual test conditions
  • Guidance from an experienced NREMT prep tutor
  • A structured NREMT prep class that teaches you to approach scenarios effectively

That’s how you prepare your mind to succeed.

Step 4: Apply for South Carolina EMT Certification

After completing the EMT training and passing the NREMT cognitive exam, apply to the South Carolina EMS Association through their online portal.

You’ll need to submit:

  • Proof of EMT course completion
  • Current CPR certification
  • NREMT cognitive exam results
  • Background check and fingerprint clearance
  • Application fee

Once your application is reviewed, you will receive your official South Carolina EMT license.

Step 5: Maintain Your EMT Certification

SC EMT certification is valid for two years. To renew, you must:

  • Keep your CPR certification up to date
  • Complete the required continuing education
  • Maintain or renew your NREMT credential

If you fail to renew, you may have to retake the NREMT, so it’s easier to stay current than to catch up later.

Why South Carolina EMTs Choose How To NREMT

At How To NREMT, we support both first-time test takers and those returning for another attempt.  About 70% of our students are taking the test for the first time and seek a clear path to success.  The remaining 30% have studied hard but didn’t earn the results they wanted. They all come to us because we show them how to think like the exam requires.
