If you’ve ever thought, “I want in,” this guide is for you, but first know this: passing the NREMT cognitive exam is a big hurdle. Many students study hard and still walk away unsure. With the right prep and support, that doesn’t have to be you.
Here’s how to get certified in Oregon and how to prepare the right way.
Step 01

Check Your Eligibility
Before training, confirm you meet these requirements:
- Be at least 18 years old
- Have a high school diploma or GED
- Hold current CPR certification (BLS for Healthcare Providers)
- Be physically and mentally ready for EMS work
- Pass a FBI fingerprint-based background check
- Provide proof of immunization, including COVID-19, TB, Vericella, and MMR tests
If you meet the aforementioned requirements, you can enroll in a state-approved EMT training program.
Step 02

Complete an Oregon-Approved EMT Course
You must complete a course accredited by the OREGON HEALTH AUTHORITY, Emergency Medical Services and Trauma Systems (EMS & TS).
These courses are available at:
- Community colleges
- EMS training centers
- Fire and rescue academies
Your training will include:
- Classroom instruction
- Skills-based labs
- Clinical ride-alongs or field shifts
This coursework prepares you for the NREMT cognitive exam.
Step 03

Pass the NREMT Cognitive Exam
The NREMT cognitive exam is where most students struggle. It’s not about memorization and what you know. It’s about putting that knowledge into practice while answering questions. This computer-adaptive test asks 70–120 questions. It ends once the system determines pass/fail. The computer-adaptive format means the difficulty level of each question varies depending how well you answer the question before it.
You’ll be tested on five updated course domains:
- Scene Size-Up and Safety
- Primary Assessment
- Secondary Assessment
- Patient Treatment and Transport
- Operations
Note: Items related to pediatric patient care are integrated throughout all domains.
Studying smarter is key. The best way to study for NREMT isn’t passive reading. You need active tools: NREMT practice exams, a guided NREMT prep class, and help from an NREMT prep tutor.
Step 04

Apply for Oregon EMT Certification
After you pass, it’s time to get state-certified. Submit your application to Oregon EMS & TS and include:
- Proof of EMT training
- NREMT cognitive exam results
- CPR certification
- Background check
- Fee payment
Once approved, you’ll receive your Oregon EMT license.
Step 05

Maintain Your Certification
Oregon EMT certification is valid for two years. To renew, you need:
- Valid CPR certification
- Completed continuing education
- Maintained NREMT certification or Oregon CE compliance
Letting your certification lapse means extra steps and possibly retesting.
